The form displays fields for a parent or guardian’s name and email, an optional phone number, a child’s age range, interest in using an Empowerment Scholarship Account (ESA), and an optional message. Age ranges are inquiry categories, not confirmed program ages.

Anything you type stays only in the current page’s memory until you refresh, navigate away, or close the page. The application does not send or save these entries, use local storage for them, or submit them to an inbox or service. Selecting “Ask About Enrollment” validates the fields but does not send an inquiry, even when all required fields are filled in.

Please avoid entering personal details until inquiries are connected. The form is intended for a parent or guardian, and does not request a child’s full name, date of birth, medical information, or ESA account details. Do not include those details in the message field.

Hosting and platform information

Although the inquiry form does not send entries, visiting a website involves requests to its hosting platform. Hosting providers may keep operational logs, such as request times, IP addresses, browser information, and requested pages, to operate and secure the site.

Platform analytics are enabled by default and may collect information about website visits and usage. This draft does not claim that the website is tracking-free. Before launch, the business owner must review the actual hosting and analytics settings and confirm the providers, information collected, any cookies or similar technologies, and available privacy choices.

What must be confirmed before launch

Before the inquiry form is connected, the business owner must confirm the submission service, who will receive inquiries, how the information will be used and protected, how long it will be retained, and how people can request access, corrections, or deletion where applicable. The owner must also confirm the appropriate privacy contact and update this notice to reflect the actual service.
